Nondiscrimination Policy Notice
St. Joseph Center for Outpatient Surgery ("Facility") complies with applicable Federal and State civil rights laws and does not discriminate on the basis of race, color, national origin, disability, source of payment, age or sex.
It is the policy of the Facility not to discriminate, exclude people or treat them less favorably because of race, color, national origin, disability, source of payment, age or sex. The Facility has adopted an internal grievance procedure providing for prompt and equitable resolution of complaints alleging any action prohibited by the Civil Rights Act of 1964, Title VI Regulation 45 C.F.R. Part 80, Section 504 The Rehabilitation Act of 1973 Regulation 45 C.F.R. Part 84, The Age Discrimination Act of 1975 Regulation 45 C.F.R. Part 90 & 91, The Education Amendments Act of 1972, Title IX Regulation 45 C.F.R. Part 86, and Section 1557 of The Affordable Care Act Regulation 45 C.F.R. Part 92.
The Facility:
Provides people with disabilities reasonable modifications and free appropriate auxiliary aids and services to communicate effectively with us, such as:
- Qualified sign language interpreters
- Written information in other formats (large print, audio, accessible electronic formats, other formats).
Provides free language assistance services to people whose primary language is not English, which may include:
- Qualified interpreters
- Information written in other languages
